Artificial intelligence is moving fast in healthcare. According to ECRI's Top 10 Patient Safety Concerns 2026 report — one of the most widely cited safety benchmarks in the industry — roughly two-thirds of physicians now report using AI in clinical practice, up from just 38% two years ago. That's a 74% increase in a single year.
Let that sink in. Not medication errors. Not staffing shortages. Not funding cuts. The biggest safety threat on the horizon is the unchecked adoption of AI tools that clinicians are increasingly — and sometimes uncritically — relying on to help make diagnostic decisions.

The Promise Is Real — But So Are the Cracks
AI's potential in diagnosis is well-documented. In radiology, AI-assisted tools have been used successfully for years. Studies show AI can reduce cognitive load, surface information faster, and in some specific contexts even outperform clinicians on narrowly defined tasks.
But the ECRI report draws a sharp line between potential and performance in the real world.
Tested machine learning models failed to detect 66% of critical or deteriorating health conditions in synthesized cases. That's not a footnote — that's a majority failure rate when it matters most.
Generative AI models showed a similar pattern: strong performance when given clean, textbook-style prompts; significantly weaker performance when dealing with realistic, open-ended patient conversations. In other words, AI performs best in scenarios that don't reflect how medicine is actually practiced.
The Risks Nobody Wants to Talk About
The ECRI report outlines several failure modes that are especially worth examining for anyone working at the intersection of AI and healthcare:
Bias baked into the model. AI learns from historical data — and historical data reflects historical inequities. Models trained on non-representative datasets can produce systematically worse results for certain patient populations, compounding existing disparities rather than reducing them.
Automation bias. There's a documented human tendency to defer to machine recommendations, even when clinical judgment says otherwise. When an AI presents a confident output, it takes an unusually self-aware clinician to push back. Most won't — and in high-pressure, high-volume clinical environments, that's not a character flaw. It's a predictable human response to a badly designed system.
Hallucinations and brittleness. AI systems are often optimized to give an answer, even when the right answer is "I don't know." In diagnostic contexts, a confident-sounding but incorrect response is more dangerous than an acknowledged gap in knowledge.
Skill erosion. Perhaps the most underappreciated long-term risk: the more clinicians offload diagnostic reasoning to AI, the weaker their independent diagnostic muscles may become. For clinicians still in training, the worry is more acute — they may never fully develop those skills at all.
Accountability gaps. As of this writing, there is still no federal regulatory framework clearly establishing liability for AI-related diagnostic errors. When something goes wrong, the question of who is responsible — the clinician, the hospital, or the AI vendor — remains unresolved.
This Isn't an Argument Against AI in Healthcare
We want to be clear: none of this is a case for slowing down AI adoption in clinical settings. AI-assisted care, done well, has the potential to meaningfully improve outcomes and reduce the cognitive burden on a workforce that is already stretched to its limits.
But "done well" is doing a lot of work in that sentence.
The ECRI report calls for a balanced approach — one that treats AI as a tool designed to supplement clinical expertise, not replace it. That framing matters. It means:
Clinicians need genuine training on AI capabilities and limitations, not just a 20-minute onboarding module
Organizations need governance frameworks that define who is responsible for AI-influenced decisions
Patients need to know when AI is being used in their care and have a meaningful right to opt out
AI-related adverse events need to be captured, reported, and fed back into improvement cycles — something that currently happens far too rarely
The Alignment Problem in Healthcare AI

The ECRI report's action recommendations point in a useful direction: AI usage policies with clear accountability structures, human factors assessments before deployment, equity-focused adoption frameworks, and training that actively builds — rather than erodes — critical thinking skills.
These aren't radical asks. They're the baseline for responsible deployment. And yet, across much of the industry, they remain the exception rather than the rule.
The organizations that get this right won't just avoid the harms the ECRI report describes. They'll build something more durable: genuine trust between patients, clinicians, and the AI systems entering the care relationship.
